Breast pain, a firm area, and fever need careful assessment. What is often called milk stasis may involve engorgement, ductal narrowing, and inflammatory swelling. Mastitis can be inflammatory or bacterial, so the same approach is not suitable for every person.
What to do before the consultation
· continue feeding in the usual pattern if it is tolerable;
· do not try to empty the breast completely, because extra pumping can increase milk production and sustain swelling;
· use a cool compress and a comfortable supportive bra for relief;
· avoid deep massage, strong pressure, and attempts to break up a lump.
If attachment is painful or the breast repeatedly becomes overfull, a lactation professional can assess positioning, latch, feeding frequency, pump use, and possible oversupply.
When medical assessment is needed
Seek medical care for marked weakness, chills, fever, increasing pain, or spreading redness. An examination is also needed when symptoms do not improve within 12 to 24 hours, a lump enlarges, or an abscess is suspected. A lactation consultant can support feeding but cannot replace medical diagnosis or treatment.
How to wean safely
Stopping suddenly increases the risk of engorgement and mastitis. Feeds are usually reduced gradually, one at a time, with time for the breasts and child to adjust. If discomfort develops, express only a small amount for comfort. The pace depends on the child’s age, feeding pattern, diet, and the parent’s breast symptoms.
