1. Appointment request
Data: patient name and age, legal representative name where applicable, phone number, selected clinic, service, doctor and preferred appointment time.
Purpose: receive the request, contact me, clarify and arrange an appointment. Operations: collection, recording, organisation, storage, clarification, use, restricted internal access, blocking, deletion and destruction, with or without automation. Retention: 180 days after submission; a canceled request is deleted 30 days after cancellation.
When booking a minor I confirm that I am their legal representative and have authority to provide the data; when booking myself I confirm the accuracy of my data.
2. Review processing and publication
The processing consent covers the author name, rating, review text and selected doctor for receiving and moderating a review. An unpublished review is deleted after 90 days.
With one separate publication checkbox I allow an unlimited circle of website visitors to access the name or alias, rating and review text I submitted, together with the selected doctor where applicable. I impose no prohibitions or additional dissemination conditions. Publication continues until I withdraw this consent or the purpose ends. I must not include a child’s identity or health information.
3. Optional marketing consent
By checking the optional marketing box, I consent to the controller using the adult applicant’s name, phone number, selected clinic direction and website source to send advertising and informational messages by phone calls, SMS and messengers. Patient age, child name, service, doctor and appointment details are not copied to the marketing list. The consent is valid for three years or until withdrawn, whichever occurs first.
4. Withdrawal
I may withdraw any consent by sending a signed request to the postal or email address above and identifying the submitted request, review or marketing contact. The controller will stop the relevant processing and delete or anonymise data unless another law requires retention.
