Vitamin D3 for children and recommended doses

Vitamin D helps the body absorb calcium and supports healthy bones. Deficiency in children can cause rickets. Taking more is not necessarily beneficial: excessive supplementation can raise blood calcium and damage the kidneys.

Vitamin D3, also called cholecalciferol, is one form of vitamin D. Choosing a product requires attention to the child’s age, health and the amount supplied in each dose.

Preventive doses used in Russia

The Russian 2021 National Programme, cited by the Union of Pediatricians of Russia, lists the following preventive doses. These are provided for discussion with a paediatrician and are not a treatment schedule for diagnosed deficiency.

Age

Preventive dose per day

Birth to under 1 month

500 IU

1 month to under 1 year

1,000 IU

1 year to under 3 years

1,500 IU

3 years to under 18 years

1,000 IU

Prematurity, obesity, kidney or liver disease and problems with absorption may require a different approach. Medicines and dietary intake also need consideration.

Recommendations differ internationally. For example, US intake targets are 400 IU daily before the first birthday and 600 IU from age one, including all sources. These figures should not be added to the Russian preventive doses. They describe different approaches to meeting vitamin D needs.

Giving vitamin D3 safely

Read the concentration on the specific bottle. One drop does not contain the same amount of vitamin D in every product. Changing brands may therefore require a different number of drops. Ask for instructions in both IU and the number of drops for the chosen preparation.

Tell the clinician about multivitamins and other supplements your child already takes. Do not increase the dose to “boost immunity” or replace a daily schedule with large occasional doses without medical advice. Do not double the next dose after a missed dose unless instructed.

Does every child need a blood test

Vitamin D status is assessed using 25-hydroxyvitamin D, written as 25(OH)D. A healthy child does not usually need this test before standard preventive supplementation. Testing may be appropriate for suspected rickets, calcium abnormalities or conditions affecting vitamin D metabolism.

Sweating, unsettled sleep or frequent colds cannot diagnose deficiency on their own. If a result is available, it needs interpretation alongside the clinical history and risk factors. Professional organisations use different thresholds; the aim is not to achieve the highest possible laboratory value.

Frequently asked questions

Should supplementation stop in summer

Yes. Discuss the duration with the child’s clinician. Deliberate unprotected sun exposure is not a safe substitute for supplementation.

Is a preventive dose also a treatment dose

Not necessarily. Diagnosed deficiency requires an individual treatment plan, including its duration and any monitoring.

At BabyClinic, parents can discuss vitamin D prevention with a paediatrician. Bring the product name, concentration and a list of all other supplements being used.
